When should I expect to receive my insurance card?
Between two to three weeks after completing your application.
How do I get a 1095 - A information ?
You can get your 1095- A information by calling the marketplace at 1-800-318-2596 and ask them to read it to you. Make sure they read it and then ask for the identifier number after you have finished getting the information read to you.
What type of health plans are available?
The health plans that are available are EPO and HMO PLANS. EPO plans allows you to visit a specialist without a referral but you must stay in network. HMO plans require you to get a referral from the primary care doctor to see a specialist.
